[Ma-pta-enotes] Personal Vehicle Use Form

Are you driving student athletes?  How about band members?  Drama students?  Any M-A student?

Then you will need to fill out the attached Personal Vehicle Use Form and take completed form to Aubrey in the AVP office.  You will also need to show your "Liability Limits" in addition to insurance, license, registration.   The "Liability Limits" can be found on the "Declarations Page" of your insurance policy. 
This must take place before you can drive students.
