[Ma-pta-enotes] Parent Driving Form

Menlo-Atherton Parents:
The district policy requires parents who use their personal vehicle to
transport students to complete and turn into the office BEFORE you drive the
district form "Personal Vehicle Use form" 
 
The form needs to be returned with copies of the Adults Driver's License and
proof of insurance.  Return the forms to the Vice Principals office as soon
as possible.  You can download the form at

Then return to Vice Principals office
