All forms needed for Carlmont’s Arena Check-In next week can be downloaded
from the website at www.carlmont.seq.org. The forms will not be mailed home this
year. 

Senior students will need to have their picture taken at Arena Check-In for the
Student Id card only. 

The membership form for our P.T.S.A. (Parent, Teacher, Student Association) is
on
the website. This year your senior student will be able to apply for a college
scholarship from the P.T.S.A. Please keep in mind that to be eligible to apply. 
your
student must be a member of Carlmont’s P.T.S.A. The membership forms and
payment ($6 per person)
will be collected at Arena Check-In. Make checks payable to Carlmont PTSA. 

As always, don't forget to bring checks or cash to purchase your yearbook and
P.A.L. 
sticker!

There are forms online if you would like to join the Carlmont Sports Boosters or
made a
donation to the Carlmont Academic Foundation

To change any information from last year's directory, or if you weren't listed
last year
and would like to be included this year, bring the downloaded form from the
website to Arena
Check-In. 

 Grad Night tickets will be on sale during the check-in process. All forms
(ticket order form, permission slip and waiver) must be filled out and have all
required
parent and students signature before your ticket purchase can be made. 

To save funds and save paper, the Bagpiper can be viewed online instead of
receiving one in the mail. This form to choose paperless is also available on
the website. 

When shopping at Office Depot for school supplies, mention Carlmont High School
and they
will donate 5% of your purchases to our school.
